Veevo Health, a service of Veevo Technologies, Inc., is on a mission to make heart attacks a thing of the past through precision health. We are deeply committed to protecting the privacy of your protected health information (PHI). PHI is information that may identify you and that relates to your past, present, or future physical or mental health or condition and related health care services. This notice describes our legal duties and privacy practices as required by the Health Insurance Portability and Accountability Act (HIPAA).

How We May Use and Disclose Your PHI

We use and disclose PHI for a variety of reasons. We have a "minimum necessary" policy, which means we only use or disclose the minimum amount of your PHI needed for the purpose.

1. For Treatment, Payment, and Health Care Operations:

• Treatment: We will use and disclose your PHI to provide you with our precision health services. This includes facilitating blood tests, analyzing results, and providing you with insights and recommendations.
• Payment: We may use your information to bill you for services.
• Health Care Operations: We use your PHI for quality assessment, business planning, and legal compliance.

2. For Research

We may use your PHI for research purposes, including training our AI models. We may also use de-identified information, which is no longer PHI and is not subject to HIPAA protections.

3. Other Permitted and Required Uses and Disclosures:

• To You or Your Personal Representative: We will disclose your PHI to you or your designated personal representative.
• Business Associates: We may share your PHI with third-party business associates who perform services for us, under a written contract that requires them to protect your PHI.
• As Required By Law: We will disclose PHI when required to do so by law.
• Public Health and Safety: We may disclose your PHI to prevent or control disease, or to prevent a serious threat to health and safety.

Your Rights Regarding Your PHI

You have the following rights regarding the PHI we maintain about you:

• The Right to Access Your PHI: You have the right to inspect and obtain a copy of your health information. To do so, you must submit a written request to our HIPAA Privacy Officer.
• The Right to Amend Your PHI: If you believe your PHI is incorrect or incomplete, you have the right to request an amendment in writing.
• The Right to an Accounting of Disclosures: You have the right to receive a list of certain disclosures we have made of your PHI.
• The Right to Request Restrictions: You have the right to request a restriction on how we use or disclose your PHI.
• The Right to Request Confidential Communications: You have the right to request that we communicate with you in a specific way or at a specific location.
• The Right to a Copy of Our Notice of Privacy Practices: You have the right to obtain a paper or electronic copy of this notice.

To exercise any of these rights, please contact our HIPAA Privacy Officer.

Changes to This Notice

We reserve the right to change our privacy practices and this notice at any time. The revised notice will be available on our website.

Complaints

If you believe your privacy rights have been violated, you may file a complaint with us or with the U.S. Secretary of Health and Human Services. To file a complaint with us, please contact our Privacy Officer. We will not retaliate against you for filing a complaint.
